結膜下に線虫を認めた結膜炎の1例

Subconjunctival nematode in a case of conjunctivitis

要約　目的：結膜下に線虫を1隻認めたため摘出し解析を行ったが，虫体の同定には至らなかった症例の報告。

症例：44歳の男性，左眼異物感で受診した。10年前にタイ，アメリカ，イタリアへの渡航歴があるも，そのほかに特記すべき既往はない。左結膜下に蠕動運動する線虫体を1隻認めたため摘出した。

所見：摘出した虫体は獨協医大熱帯寄生虫病教室で解析。形態学的には旋尾線虫目に属する線虫の雄と判明したが，種の同定には至らなかった。

結論：特記すべき既往がなくても結膜下に寄生虫を認める症例があり，原因不明の結膜炎症例の鑑別の1つとして念頭に置く必要があると思われた。

Abstract　Purpose：To report a case of conjunctivitis with subconjunctival nematode.

Case：A 44-year-old male presented with foreign body sensation in his left eye since the day before. He had visited Thailand, Italy and USA 10 years before. He had no history of keeping domestic cat or dog.

Findings and Clinical Course：The left eye showed a moving transparent worm in the subconjuntival space temporal to the cornea. The worm was removed under observation by a slit-lamp microscope. It was identified as a male belonging to the order Spirurida.

Conclusion：This case illustrates that subconjunctival parasite may cause conjunctivitis of apparently unknown cause.
