Abstract Purpose: To report the outcome of two methods of surgery for primary pterygium.

Cases and Method: This retrospective study was made on 65 eyes of 49 patients who received pterygium surgery in the past 8 years and who were followed up for 6 months or longer. A group of 51 eyes of 36 patients received excision of pterygium and autograft of bulbar conjunctiva. The other group of 14 eyes of 13 patients received Z-plasty without excision of pterygium. Recurrence was defined as neovascularization into the cornea.

Results: Pterygium recurred in 7 eyes(13.7%)following excision and autograft of bulbar conjunctiva. Four of the 7 eyes needed additional surgery. Pterygium recurred in 3 eyes(21.4%)following Z-plasty. One of the 3 eyes needed additional surgery.

Conclusion: Rate of recurrence was the same following bulbar conjunctival autograft or non-excising Z-plasty for primary pterygium.
