緑内障に対する選択的レーザー線維柱帯形成術の既往がリパスジル点眼の眼圧下降効果に及ぼす影響

Influence of selective laser trabeculoplasty on the effect of ripasudil for glaucoma

要約　目的：緑内障に対する選択的レーザー線維柱帯形成術（SLT）の既往がリパスジル点眼の眼圧下降効果に与える影響の報告。

対象と方法：通院加療中の開放隅角緑内障24例35眼を対象とした。男性18眼，女性17眼で，年齢は68.9±9.8歳である。点眼スコアは3.9±0.7であった。全員にリパスジルの追加点眼を開始した。18眼にはSLTの既往があり，17眼にはなかった。

結果：SLT既往群では，リパスジル点眼追加前の眼圧は15.1±4.6mmHgで，6か月後の眼圧は13.8±4.5mmHgであり，有意差はなかった（p＝0.12）。SLT非既往群では，リパスジル点眼追加前の眼圧は17.8±5.7mmHgで，6か月後の眼圧は14.8±3.6mmHgであり，有意に下降した（p＜0.01）。眼圧下降率は，SLT群と非SLT群の間に有意差はなかった。眼圧下降率が10％以上の有効例は，SLT群で11眼（61.1％），非SLT群で10眼（58.8％）であり，両群間に有意差はなかった。

結論：SLTの既往眼では，リパスジル点眼による眼圧下降効果が低かった。

Abstract　Purpose: To report the influence of selective laser trabeculoplasty（SLT）on the effect of topical treatment with ripasudil for glaucoma.

Cases and Method: This retrospective study was made on 35 eyes of 24 patients who had been treated for open-angle glaucoma. The series comprised 18 male and 17 female eyes. The age averaged 68.9±9.8 years. Medication score averaged 3.9±0.7. All the cases started additional topical treatment with ripasudil. Eighteen eyes had received SLT before.

Results: In 18 eyes with prior SLT, intraocular pressure（IOP）averaged 15.1±4.6 mmHg at the start of ripasudil and 13.8±4.5 mmHg 6 months later. The difference was not significant（p＝0.12）. In 17 eyes with no history of SLT, IOP averaged 17.8±5.7 mmHg at the start of ripasudil and 14.8±3.6 mmHg 6 months later. The difference was significant（p＜0.01）. There was no significant difference in the rate of IOP decrease between the two groups.

Conclusion: Additional instillation of ripasudil showed lesser ocular hypotensive effect in eyes that had received SLT in the past.
