全身浮腫による体重増減と並行した漿液性網膜剝離を繰り返したネフローゼ症候群の1例

A case of nephrotic syndrome with recurrent serous retinal detachment synchronized with weight gain related to systematic edema

要約　目的：漿液性網膜剝離を呈したネフローゼ症候群の症例報告。

症例：59歳男性が左眼の視力低下で受診した。1年前にネフローゼ症候群と診断されていた。全身浮腫のため体重が10kg増加していた。

所見と経過：矯正視力は右1.2，左0.6であった。左眼黄斑部に漿液性網膜剝離を認めた。利尿薬投与とステロイドパルス治療の後，体重減少と同期して網膜剝離は改善し，1週間後の左矯正視力は1.2であった。その後，漿液性網膜剝離は18か月の間で2回再発し，全身的治療（塩分制限や透析導入）と体重減少の後に速やかに改善した。

結論：ネフローゼ症候群による漿液性網膜剝離は体重増減と同期していた。

Abstract　Purpose: To report a case of nephrotic syndrome with recurrent serous retinal detachment.

Case: A 59-year-old male presented with impaired visual acuity in the left eye. He had been diagnosed with nephrotic syndrome 1 year before. He gained 10 kg in weight with systemic edema.

Findings: Corrected visual acuity was 1.2 right and 0.6 left. The left eye showed serous retinal detachment in the macula. After he was treated for systemic edema, improvement of retinal detachment was synchronous with weight loss, visual acuity was 1.2 left a week later. Subsequently, serous retinal detachment associated with systematic edema recurred two times for 18 months, and improved quickly after treatment and weight loss.

Conclusion: Serous retinal detachment caused by nephrotic syndrome was synchronous with weight gain and weight loss.
