Abstract Purpose: To report a case of papilledema as manifestation of chronic subdural hematoma and intracranial hypotension syndrome.

Case: A 30-year-old female presented with photophobia and metamorphopsia. She had been diagnosed with papilledema and chronic subdural hematoma 2 months before.

Findings: Corrected visual acuity was 1.0 in either eye. Both eyes showed prominent papilledema, decreased critical flicker frequency, and enlarged blind spot. Magnetic resonance imaging(MRI)showed chronic subdural hematoma suggestive of intracranial hypotension syndrome. She had also orthostatic headache. Subdural hematoma and papilledema disappeared after 2 months of conservative treatment with improved subjective symptoms.

Conclusion: The present case illustrates that papilledema may be due to chronic subdural hematoma with intracranial hypotension syndrome.
