低髄液圧症候群に伴う慢性硬膜下血腫に視神経乳頭浮腫を合併した1例

Papilledema in a case of intracranial hypotension syndrome with subdural hematoma

要約　目的：低髄液圧症候群に伴う慢性硬膜下血腫に視神経乳頭浮腫が生じた症例の報告。

症例：30歳の女性が羞明と変視症で受診した。2か月前に慢性硬膜下血腫と視神経乳頭浮腫として診断されていた。

所見：矯正視力は左右眼とも1.0で，顕著な視神経乳頭浮腫が両眼にあった。中心フリッカ値が低下し，盲点の拡大があった。MRIで慢性硬膜下血腫があり，低髄液圧症候群の可能性が指摘された。眼症状以外に起立性頭痛があった。保存的な治療で，2か月後に硬膜下血腫と視神経乳頭浮腫は消失し，自覚症状も改善した。

結論：若年者での視神経乳頭浮腫では，慢性硬膜下血腫が原因となり，低髄液圧症候群が関与している可能性がある。

Abstract　Purpose: To report a case of papilledema as manifestation of chronic subdural hematoma and intracranial hypotension syndrome.

Case: A 30-year-old female presented with photophobia and metamorphopsia. She had been diagnosed with papilledema and chronic subdural hematoma 2 months before.

Findings: Corrected visual acuity was 1.0 in either eye. Both eyes showed prominent papilledema, decreased critical flicker frequency, and enlarged blind spot. Magnetic resonance imaging（MRI）showed chronic subdural hematoma suggestive of intracranial hypotension syndrome. She had also orthostatic headache. Subdural hematoma and papilledema disappeared after 2 months of conservative treatment with improved subjective symptoms.

Conclusion: The present case illustrates that papilledema may be due to chronic subdural hematoma with intracranial hypotension syndrome.
