Abstract Background:This report presents the case of a patient who developed secondary glaucoma after penetrating keratoplasty(PKP)for corneal opacity associated with familial lecithin-cholesterol acyltransferase(LCAT)deficiency that improved with trabeculotomy ab interno.

Case:A 65-year-old man undergoing hemodialysis for renal failure associated with LCAT deficiency.

Findings:The initial visual acuity was 0.9 OD and 1.2 OS. Corneal stromal opacities were detected in both eyes. PKP was performed to halt the progression of blurring in the right eye. Intraocular pressure(IOP)increased on postoperative day 3, and exhibited resistance to treatment. The iris root was attached to the trabecular meshwork around the anterior chamber angle;however, peripheral anterior synechiae were not observed. Lens reconstruction and trabeculotomy ab interno were performed on postoperative day 43. No increase in IOP was observed month 7 postoperatively.

Conclusions:Trabeculotomy ab interno has a minimal impact on the corneal endothelium. Thus, it may be an effective treatment for secondary glaucoma following PKP.
