急性脈絡膜循環障害に点状螢光漏出と脈絡膜隆起をきたした症例

A case of acute choroidal ischemia with punctate dye leakage and choroidal detachment

　脈絡膜虚血部に螢光漏出点を生じ，さらに脈絡膜隆起を伴った三角症候群の新鮮例を報告した。症例は71歳男性で，右眼の白内障手術後，経過は良好であったが，2週目に変視症と軽度の視力低下をきたした。右眼底の黄斑部から上耳側にかけ限局的な脈絡膜隆起と漿液性網膜剥離がみられ，螢光眼底で島状の脈絡膜充盈遅延域とその後極側に色素漏出点を認めた。これらの変化は徐々に改善され，経過とともに脈絡膜虚血域の中心部に帯状の萎縮巣が形成された。螢光上中心性脈絡膜症にしばしば観察される芯を持つ過螢光巣を認めた。本症例の発症機序を以下のように考えた。強度の脈絡膜虚血により，網膜色素上皮は萎縮性瘢痕を生じ，軽度の虚血では関門機能が障害された。また，渦静脈の逆流による脈絡膜の鬱血腫脹，上脈絡膜腔への漏出液貯溜により，脈絡膜隆起を生じたものであろう。本症例により，中心性脈絡膜症の原因として，脈絡膜循環障害の関与が示唆される。

A 71-year-old man developed metamorphopsia and blurred vision in his right eye 2 weeks after cataract extraction and intraocular lens implanta-tion. We observed choroidal detachment with ser-ous retinal detachment in the superior temporal quadrant inclusive of the macula. Fluorescein an-giography showed choroidal filling delay in the affected sector as well as leakage spots along itsposterior margin.

Spontaneous improvement resulted in the chor-oidal detachment during the ensuing 2 weeks, leav-ing band-shaped atrophy of the retinal pigment epithelium in the central area of choroidal is-chemia. Fluorescein angiography showed dark spots with hyperfluorescent halo as is commonly observed in central serous choroidopathy. This case seemed to be in favor of the claim that choroidal ischemia is involved in the pathogenesis of central serous choroidopathy.
