急性脈絡膜循環障害

Acute choroidal ischemia

　螢光眼底所見上明らかに脈絡膜螢光流入遅延の証明された3症例を報告した。成因として症例1は海綿静脈洞血栓症，症例2は原因不明の後毛様動脈循環不全，症例3は悪性高血圧症が各々考えられた。急性期の脈絡膜循環障害を螢光造影所見にて臨床的にとらえた報告は稀である。さらに全例で脈絡膜螢光流入遅延部に隣接して漿液性網膜神経上皮剥離が明らかに確認された。全例で急性期に患眼の網膜EOGのLp／Dt比が低値となり螢光造影所見と共に脈絡膜循環障害を反映する良い指標となった。

We observed 3 cases with serous detachment of the retinal neuroepithelium and manifest choroidal filling delay on fluorescein angiography. As the causative lesion, there was unilateral cavernous sinus thrombosis in case 1, unilateral posterior cili-ary artery occlusion of unknown origin in case 2and malignant hypertension in case 3.

The pattern of choroidal hypofluorescence was geographical and was widely distributed over the posterior pole in the first 2 cases. Case 3 manifested persistent small solitary choroidal hypofluorescence temporal to the macula.
