I evaluated the integrity of blood aqueous barrierby means of fluorescein iris angiography and ante-rior fluorophotometry. A series of 376 eyes withoutretinopathy in 198 diabetics and another series of217 eyes with early retinopathy, grade Al afterFukuda, in 114 diabetics served as material in thestudy.

Fluorescein iris angiography showed a higherincidence of dye leakage along the pupillary mar-gin in both serieses than in normal controls. Pupil-lary dye leakage was present even in youngerpatients under 40 years of age. Its incidence waslarger in eyes with retinopathy than in those with-out.

Anterior fluorophotometry showed an increase ofaqueous/plasma ultrafiltrate fluorescein concentra-tion ratio (Fa/Fp ratio) before appearance ofretinopathy. It increased with advance in age andretinopathy. It increased in poorly controlled dia-betics with HbA1c less than 7% and in patientstreated with insulin.

The findings suggested that the blood-aqueousbarrier is affected in diabetic subjects prior tomanifest diabetic retinopathy and under 40 years ofage. The disturbance in the barrier was acceleratedwith advance in age, manifest retinopathy and highHbA1c, level.
