We evaluated a consecutive series of 265 eyes who underwent cataract surgery with posterior chamber lens (IOL) implantation during the fore-going 45 months. The eyes were followed up for 3 months or more postsurgically.

Postoperative fibrinous exudation occurred in 19 eyes (7.2%). We noted two peaks as to its onset: the 1st postoperative day and the 4th to 5th day. The exudation subsided before the 7th postoper-ative day except in 3 eyes. Posterior synechia developed in 4 out of the 19 eyes (21%). Pupil capture developed in one eye. Final visual acuity of 20/20 was obtained after repositioning.

As an overall estimate, posterior synechia was observed in 38 eyes (14.3%) under full mydriasis. The iris was adherent to the surface of IOL in 19 eyes and to the residual lens capsule in 9. The visual acuity was not influenced by the synechia. No correlation was present between the synechia and the make or type of IOL. Instillation of mydriatics for a prolonged period seemed to be necessary to prevent occurrence of posterior synechia.
