偽性Foster-Kennedy症候群を呈した放射線視神経症の1例

A case of radiation optic neuropathy manifesting pseudo-Foster-Kennedy syndrome

要約　目的：偽Foster-Kennedy症候群を呈した放射線視神経症の1例について報告する。症例：65歳女性が右視力低下で受診した。矯正視力は右（0.01），左（0.8）で右眼の相対的瞳孔求心路障害（RAPD）陽性であった。眼底検査において右視神経乳頭周囲網膜線維層の菲薄化，左視神経乳頭腫脹を認め，Foster-Kennedy症候群様の病態を呈したが，頭蓋内圧亢進はなかった。動的視野検査において右眼に中心暗点を検出した。眼科受診の3か月前に他院で乳癌術後再発による骨・脳転移と診断され，放射線全脳照射を受けていた。これらの臨床経過より，全脳照射による放射線視神経症が考えられた。結論：偽性Foster-Kennedy症候群の原因疾患の1つに全脳照射後の放射線視神経症がある。

Abstract.　Purpose：To report a case of radiation optic neuropathy manifesting pseudo-Foster-Kennedy syndrome. Case：A 65-year-old woman presented with impaired vision in the right eye of recent onset. She had been diagnosed with breast carcinoma 11 years before followed by excision. Cerebral and skull metastases were detected 4 months before and were treated by radiation of the whole brain totalling 50 Gy. Findings：Corrected visual acuity was 0.08 right and 0.8 left. The right eye showed attenuation of retinal nerve fiber layer around the disc. The left eye showed swelling of optic disc. The findings simulated Foster-Kennedy syndrome. Conclusion：Pseudo-Foster-Kennedy syndrome in the present case seemed to be manifestation of optic neuropathy secondary to whole brain radiation.
