レーザー虹彩切開術後に進行した網膜前黄斑線維症の1症例

Preretinal macular fibrosis progressed after laser iridotomy

（1）アルゴンレーザーとYAGレーザーの併用による虹彩切開術後，網膜前黄斑線維症が進行した閉塞隅角緑内障の一症例を報告した．（2）硝子体手術により網膜前線維膜は除去され，視力・変視症は改善したが，術中所見で虹彩切開部位の後方の硝子体内に多数のアルゴンレーザー虹彩切開術の際に虹彩上皮細胞あるいは実質細胞が硝子体内に散布され，増殖を起こして網膜前黄斑線維症に至ったものと推察された．

A 74-year-old woman was treated by argon laser iridotomy for chronic angle closure glaucoma in her left eye. YAG laser was additionally applied 8 days later to perforate the iris. Metamorphopsia developed after another 6 days.

When seen by us 7 months later, she manifested, funduscopically, preretinal macular fibrosis in her left eye. Pigment cellular proliferation was obser-ved in the anterior vitreous.

As a possibility, laser irradiation induced disper-sion of the pigment epithelial and stromal cells of the iris intor the vitreous. Proliferation of these cells would have led to formation of preretinal membrane on the macula. We treated her by vitrectomy and peeling of the membrane with favorable outcome.
