Bromocriptine内服により著明な視機能の改善が得られたprolactinomaの4例

Systemic bromocriptine improved visual functions in 4 cases with macroprolactinoma

　種々の理由から手術が行えず，Bromo-criptineによる治療を行い，視機能の著しい改善が得られたprolactinomaの4症例を報告した．

　症例　1は16歳女性で，右眼の著しい視力低下が認められた．投薬7カ月後に視機能は著明に回復した．症例2は28歳女性で，頭痛をきっかけとして発見され，右同名半盲が認められた．投薬3カ月後には視機能は著明に改善した．症例3は55歳男性で，右眼耳側視野欠損で発症した．内服により視機能は徐々にではあるが改善が認められた．症例4は39歳男性で，両眼視力低下があり，両耳側視野欠損が認められた．投薬10日後には視機能は正常に回復した．

　Bromocriptine単独治療で視機能の著しい回復が得られる症例があることがわかった．

　さらにBromocriptineの作用機序について考察し，手術症例と比較検討した．

We treated 4 cases of pituitary prolactinoma by systemic bromocriptine. The initially impaired visual acuity recovered in all the affected 4 eyes. Visual field was affected in 7 eyes. It either im-proved or completely recovered in all the cases. All the cases in the present series belonged to more advanced stages of prolactinoma with extrasellar extension of the tumor and with serum prolactin level of 1,000 ng/ml or more. Our findings indicate that systemic bromocriptine may induce marked improvement in visual acuity and visual field in advanced cases of macroprolactinoma.
