川崎病の発病早期からの眼病変

Course of ocular manifestations of Kawasaki disease

　川崎病患者44例について発病早期から少なくとも週1回眼科的検査を行った．

　眼球結膜充血は平均3.7病日に44例全例の両眼に出現，平均7.4日間持続した．虹彩炎は第1病週から観察しえた36例中26例（72％）に認められ，この26例中10例では眼科初診時に川崎病と確定診断されていなかった．虹彩炎の発現率は第2病週以降急減し，第6病週までに全て後遺症を残さず消退した．虹彩炎は全て両側性で女よりも男で高頻度にみられた．眼底検査，螢光眼底検査で明らかな異常を認めた例はなかった．冠動脈瘤を認めた6例では虹彩炎が第1病週から認められ，しかも眼球結膜充血，虹彩炎とも長期間持続する傾向がみられた．

　川崎病の虹彩炎は，発病早期から高率に出現し，冠動脈瘤を認める例で虹彩炎が遷延化する傾向がみられた．虹彩炎を川崎病の主要症状の一つとして考えれば，川崎病の早期診断および重症度評価に有用であると考えた．

I evaluated the ocular manifestations of Kawasa-ki disease in 44 patients. The age of patients ranged from 3 to 75 months, average 23 months. All the cases were seen from the onset of the disease and were followed up one a week or more.

All the patients manifested bilateral nonex-udative bulbar conjunctivitis from the 1st to 7th day on and for a duration of 1 to 19 days. Early in the course of the disease, 32 patients (73%) developed bilateral iritis. The incidence of iritis was 85% for males and 63% for females.
