A 75-year-old, otherwise healthy male underwent extracapsular cataract extraction and posterior cham-ber intraocular lens implantation surgery in his right eye. Mixture of mydriatic solution was instilled the next day, which resulted in persistent to maximal pupil-lary dilatation, non-responsive to any miotics.

One year later, his left eye was operated by the same surgeon, with the same procedure and the same intraocular lens as the previous one. This time, the pupil of the left eye responded well to mydriatics and miotics. No causative factor could be identified responsible for the irreversible mydriasis in the right eye.
