Out of a consecutive series of 248 eyes who under-went planned extracapsular cataract extraction with posterior chamber intraocular lens implanta-tion during the foregoing 3-year period, we obser-ved 8 eyes which developed irreversible mydriasis. Typically, it occurred 3 to 5 days after surgery and progressed gradually during the ensuing few weeks. The pupil diameter reached maximally 7.5 to 8.0 mm. The pupil did not respond to any topicalmiotics.

We could not identify the exact cause for this unusual complication, but its clinical course seemed to resemble Urrets-Zavali a syndrome which develops after corneal transplantation. Ischemia of the iris vessels might play a role in this complica-tion, as it would induce paralysis of the pupil sphinc-ter muscles leading to irreversible mydriasis.
