人工水晶体挿入後の緑内障の1例

A case of glaucoma in pseudophakia

　後房レンズ挿入後，高度の隅角色素沈着によるものと思われる緑内障で，眼圧のコントロールが困難であった1例を経験した。

　後房レンズ挿入後の27名27眼の隅角検査を施行した結果，術眼にScheie I〜IV度の種々の程度の隅角色素沈着を全例に認め，いずれも非手術眼である反対眼に比較して高度であった。内，緑内障となったのは1眼であり，最も色素沈着が高度であった。

　したがって，後房レンズ挿入による隅角色素沈着は多くの場合，眼圧上昇をもたらさないが，高度の場合は緑内障を惹起しうるものと考えられる。

　また，このような緑内障に対し，laser trabeculoplastyが眼圧コントロールに有効であった。

Secondary glaucoma developed following cata-ract surgery and implantation of posterior chamber lens in a 68-year-old male patient. The intraocular pressure started to rise 3 weeks after uneventful surgery described above in his right eye. Gonio-scopy revealed massive pigment accumulation in the right eye only. Trabeculectomy and laser trabec-uloplasty finally brought the intraocular pressure under control.
