水晶体吸引術中の散瞳保持

Maintenance of Mydriasis during Cataract Aspiration Surgery

緒　論

　軟性白内障に対する手術術式はScheie^1）の発表以来，水晶体吸引術が主として行なわれるようになり，種々の方法，器具が考案されて来た。我我はBarraquer^2）の術式を基にした，灌流および吸引用の2種類のカニューレを使つて吸引術を行なつている^3）。多くの著者の一致した強調点は術中の十分な散瞳の保持である。術後の合併症の主体をなす虹彩後癒着とこれにともなう瞳孔変形は，水晶体を十分吸引することにより除去できる問題であり，かつこれを可能ならしめるのは術中の散瞳保持と考えられる。

　しかし我々は吸引を行なつている間に縮瞳を来し虹彩のうしろにある水晶体を吸引することが困難であり，かつ危険となる例を数々経験する。再度散瞳させることはアトロピン等を使用しても不可能である。こうした際の処置についてGirard^4）は1,000倍エピネフリンを灌流液中に滴下するとよいと述べている。

It is imperative to maintain mydriasis during aspiration surgery of soft cataract. Through our consecutive of 48 aspiration operations without topical indomethacin, miosis often occurred during surgery. This miosis was characterized by atropine resistance. We tho-ught that prostaglandins participated in this phenomenon. In addition to routine applications of mydriatics, 23 eyes were operated with instillation of 0.5 percent indomethacin sesame oil preparation 4 times before surgery, which was known to inhibit prostaglandin synthesis.
