高齢者における人工水晶体移植術後の視機能—とくに術後乱視について

Change of corneal astigmatism after intra ocular lens implantation in aged patients

　平均74.5歳の高齢者の117眼に人工水晶体移植を行い，術後3カ月間の視力と乱視の変化を調べた。術後3カ月で，86％が0.5以上の矯正視力を得た。ほとんどの症例が，術直後は強い直乱視を示すが，時間経過につれ乱視度は軽減し，倒乱視の方向に変化した。倒乱視化の傾向は，特に前房レンズ二次移植の例で著しかった。また，縫合糸および縫合方法の工夫により，術後乱視を軽減することができた。

　We evaluated the changes in visual acuity and corneal astigmatism during the 3 months after intra ocular lens implantation in 117 eyes. The ages of the patients ranged from 50 to 97 and averaged 74.5 years. Posterior chamber lens was implanted following phacoemulsification in 55 eyes and following planned extracapsular cataract extraction in 37 eyes. Anterior chamber lens was implanted in 25 aphakic eyes. Measurements of visual acuity, refraction and corneal astigmatism were performed after 1 week, 1 month and 3 months after surgery.
