Petriellidium boydiiによる角膜真菌症の1例

A case of Keratomycosis by Petriellidium boydii

　角膜を穿孔し水晶体内に膿瘍形成をきたしたきわめて稀な真菌Petrieltidium bqydii（＝Allescheria boydii）感染症の1症例を報告した。症例は47歳男子で木の枝で眼を突いた後に難治性角膜潰瘍が発生し，Amphotericin Bを局所および全身投与したにもかかわらず症状が著しく悪化したため，患眼は発症後約2ヵ月後に摘出された。摘出眼球のPAS染色標本では，水晶体前嚢下に菌糸と少数の胞子様構造物を確認した。P．boydiiによる角膜真菌症は本邦第3例目である。

A rare case was presented of keratomycosis in a 49-year-old male who was accidentally inoculated by the organism when stabbed in the right eye with a tree spray. A corneal ulcer soon developed and it resisted our treatment with high doses of broad-spectrum antibacterial agents. Since the clinical picture and course suggested a fungal infection, Amphotericin B solution (0.3%) was frequently ap-plied to the affected eye. On recognition of the positive culture of fungus from the corneal scrap-ings, systemic administration of Amphotericin B was also started.
