Abstract. Purpose:To report of case of severe keratomycosis treated by voriconazole. Case:A 63-year-old woman developed corneal ulcer in her right eye. She was referred to us after treatment proved futile for 17 days. Findings:Her visual acuity was hand motion right and 1.2 left. The right cornea was opaque over all the surface with hypopyon and corneal ulcer. Fusarium sp was isolated from corneal scrapings. She received intravenous voriconazole, pimaricin ointment, and hourly instillation of 0.2% voriconazole. Corneal ulcer started to improve after start of instillation of 1% voriconazole on day 11. Penetrating keratoplaty had to be performed for traumatic corneal rupture on day 17. The obtained corneal specimen showed only a few fungi. Conclusion:This case illustrates that instillation of 1% voriconazole may be effective for keratomycosis due to Fusarium sp.
