ボリコナゾールが奏効した角膜真菌症の1例

Voriconazole was effective in a case of keratomycosis

要約　目的：重篤な角膜真菌症に対して抗真菌薬ボリコナゾールを使用した症例の報告。症例：63歳女性の右眼に角膜潰瘍が生じた。17日間の治療が奏効せず，当科を紹介された。所見と経過：視力は右手動弁，左1.2で，右眼角膜のほぼ全面が混濁し，前房蓄膿と大きな角膜潰瘍があった。病巣からフサリウム属が検出された。ボリコナゾール点滴，ピマリシン眼軟膏，0.2％ボリコナゾールの1時間毎の点眼を開始した。第11病日からボリコナゾール点眼の濃度を1％に変更してから角膜潰瘍が軽快しはじめた。第17病日に打撲により眼球破裂が生じ，全層角膜移植を行った。得られた角膜に真菌は一部のみにあった。結論：フサリウムによる角膜真菌症に1％ボリコナゾールの点眼が奏効することがある。

Abstract.　Purpose：To report of case of severe keratomycosis treated by voriconazole. Case：A 63-year-old woman developed corneal ulcer in her right eye. She was referred to us after treatment proved futile for 17 days. Findings：Her visual acuity was hand motion right and 1.2 left. The right cornea was opaque over all the surface with hypopyon and corneal ulcer. Fusarium sp was isolated from corneal scrapings. She received intravenous voriconazole, pimaricin ointment, and hourly instillation of 0.2％ voriconazole. Corneal ulcer started to improve after start of instillation of 1％ voriconazole on day 11. Penetrating keratoplaty had to be performed for traumatic corneal rupture on day 17. The obtained corneal specimen showed only a few fungi. Conclusion：This case illustrates that instillation of 1％ voriconazole may be effective for keratomycosis due to Fusarium sp.
