高度の網膜血管炎を呈した全身性エリテマトーデスの1例

Severe bilateral retinal vasculitis in a case of systemic lupus erythematosus

　両眼の激しい網膜血管炎を呈し，高度の網膜血管閉塞をきたした全身性エリテマトーデス（SLE）の1例（38歳男性）を報告した。

　初診時，軽度のぶどう膜炎を伴い，両眼底には著明な網膜静脈周囲炎が認められたが，3日後には静脈閉塞症様の強い網膜出血をきたし，同時に網膜剥離を併発した。網膜動脈周囲炎も数日遅れて明らかとなった。

　ステロイド大量投与により，眼底の著しい滲出性病変および多彩な全身症状は速やかな改善を示し，血管炎も鎮静化したが，罹患網膜動静脈の多くは高度の閉塞を残した。

　全身的にも皮膚，筋，腎の各生検で壊死性血管炎が認められた。本症例の病初期の眼底像は実験的アレルギー性網膜血管炎に類似しており，両限の著明な綱膜血管炎の発症には，SLEに基づいた免疫学的機序の関与が推察された。

A 38-year-old male with systemic lupus erythematosus (SLE) developed severe retinal vasculitis in both eyes. The initial ocular findings were characterized by marked retinal periphlebi-tis and diffuse retinal edema accompanied with mild uveitis. On subsequent visit 3 days after, bilateral fundus lesions made abrupt changes show-ing extensive retinal hemorrhages, branch vein occulusion, exudative retinal detachment and intraretinal exudates around the affected venules especially in the macular region. Perivasculitis appeared in some retinal arterioles later.
