先天性チアノーゼ性心疾患の眼底

Retinopathy in cyanotic congenital heart disease

　先天性チアノーゼ性心疾患患者211名の眼底を精査し，次の結論を得た。

（1）二次性多血症の程度と動静脈の拡張・静脈の蛇行率が相関していたので，眼底変化は多血症によるものと考えられた。

（2）網膜中心動脈閉塞症・周辺血管白鞘形成などの眼底変化が，二次性多血症の強い症例（赤血球数800万／mm³以上）にみられた。

（3）年齢との関係では，年齢と血管径・蛇行率に相関は認められなかった。

（4）脳血管障害を合併した症例では，強い眼底変化をみるものが多いが，乳頭の発赤・腫脹は必ずしも，脳病変の存在を示唆するものではなかった。

A consecutive series of 211 patients with con-genital cyanotic heart disease were evaluated as to the fundus manifestations. The ages of the patients ranged from 1 month to 43 years.

The severity of fundus changes, particularly dila-tation of retinal vessels and tortuosity of retinal veins, were closely related to the degree of secon-dary polycytemia.

Grave ocular complications, as occlusion of retinal artery of retinal vein obstruction, were gene-rally associated with severe polycytemia (RBC over 800 X 10⁴/mm³).
