遠隔転移による海綿静脈洞腫瘍17症例の臨床的検討

Clinical features of seventeen cases of cavernous sinus tumor due to distant metastasis

（G-8PM-7）　日本大学医学部附属板橋病院を主とした施設で，海綿静脈洞の占拠性病変が画像診断で検出され，原発巣も確認された片側性の転移性海綿静脈洞腫瘍17症例の臨床像および画像診断の特徴について検討した。年齢は15歳から72歳の男性9例，女性8例であった。全例眼筋麻痺を示し，13例で三叉神経障害を合併した。視神経障害は4例でみられ、全例全外眼筋麻痺となった。7例が瞳孔不同を示した。CTおよびMRIでは転移巣が海綿静脈洞の外壁の突出像あるいは洞部の腫大像として観察できた。

We reviewed 17 cases of unilateral cavernous sinus tumor due to distant metastasis of malignancy. The series comprised 9 males and 8 females. Their ages ranged from 15 to 72 years, average 56 years. All the cases showed external ophthalmoplegia. Trigeminal nerve palsy was present in 13 cases. The optic nerve was involved in 4 cases. All the 4 cases resulted in total ophthalmoplegia. Anisocoria was present in 7 cases. Computed tomography and magnetic resonance imaging showed either lateral bulging of the wall or space-occupying enlargement of the cavernous sinus.
