低眼圧緑内障のステロイド反応

Steroid response in low tension glaucoma

　いわゆる広義の低眼圧緑内障とされている症例の中には健常眼圧を越えた真の低眼圧緑内障と偽緑内障とがある。これらの鑑別を目的として，広義の低眼圧緑内障患者のステロイド反応，視神経乳頭線状出血などを調べた。

　対象は両眼性の緑内障性視神経乳頭とこれに相応する視野障害を有する50名100眼である。視野障害（湖崎）はIIa〜IIIa期が91％を占めた。ステロイド負荷試験では，陽性は36眼でこのうち強陽性は17眼であった。強陽性眼の視神経乳頭の陥凹型は血管の鼻側偏位をもつ陥凹拡大型とnotching形成型が多かった。視神経乳頭出血は全体では30％に，6ヵ月以上経過観察できた症例では37％にみられた。出血とステロイド反応の間に相関関係はなかった。以上から真の低眼圧緑内障と偽緑内障との明確な鑑別は困難であるが，血液循環障害が，前者ではわずかな眼圧上昇と相まって，後者では単独に陥凹形成に関与していると推測された。

One hundred eyes of fifty patients with so-called low tension glaucoma (LTG) was studied as to the differentiation between genuine LTG and pseudo-glaucoma with special reference to steroid response, splinter disc hemorrhage and cupping formation.

All the subjects had bilateral glaucomatous optic disc changes and related visual field defects with mostly II a to III a stage by Kosaki's classification. In a corticosteroid provocative test, 36 out of 100 eyes were positive, of which 17 eyes showed high positive response.
