実質型角膜ヘルペスの免疫療法—第3報levamisole®による臨床効果とくに長期観察例を中心に

Immunotherapy for the herpetic stromal keratitis.:Report 3. Long-term results of levamisol®therapy

　実質型角膜ヘルペス患者28例を対象としてL-tetramisolc （levamisole®）による免疫療法を行い，4年以上経過を観察しえた長期観察例も含め，本療法の臨床効果および問題点について検討した。

　投与量は1週間450mgの3投4休法，あるいは1週間150mgの1投6休法にて行い，最短3カ月から最長1年5カ月間の薬剤投与を行った。

　視力の改善したものは3カ月後68％，6カ月後73％，1年後61％であったが，症状別に検討した結果からは潰瘍を伴った軽度浮腫群に著効例が多かった。

　再発率は1年後19％，2年後27％，3年後33％，4年後20％であり，IDU・steroid併用療法に比べ低率を示した。

　副作用は発熱・発疹・掻痒感・臭覚異常・腹痛などで，いずれも投薬中止にて治まり，無顆粒球症はみられなかった。

Twenty-eight patients with herpetic stromal keratitis were treated with oral administration of levamisole and were followed up to a maximum of 4 years. Twenty-four of them were given 150 mg of levamisole a day for 3 consecutive days once every week and the remaining 4 were given 150 mg of levamisole once weekly. The visual acuity improved in 43% of the patients in one month after treat-ment, 68% in 3 months, 73% in 6 months and 61% in 12 months.
