膨隆水晶体による急性閉塞隅角緑内障

Acute angle closure glaucoma due to swollen lens

（1）白内障の未熟期，成熟期ないし過熟期に水晶体が膨張して水晶体前面が前進し，pupillary blockをおこして急性閉塞隅角緑内障による急性発作が発生することがある。

　このような膨隆水晶体による急性閉塞隅角緑内障を最近5年間に13例経験した。これはこの間の原発性閉塞隅角緑内障の1／5の症例数で，従来の報告^3〜5）以上に高頻度であった。

（2）平均年齢は76歳と高齢者が多く，成熟白内障が大多数をしめた。

（3）他眼の前房は半数が浅前房であり，元来浅前房の眼に白内障で水晶体が膨隆すると本症が発生しやすい。

（4）寿命延長に伴い，片眼白内障の高齢者がふえる結果，本症はますますふえると思われるので，急性緑内障発作時に本症を見逃さないように注意を要する。

（5）本症は高齢者に多く，緑内障発作による全身への悪影響，高浸透圧剤，炭酸脱水酵素阻害剤使用による全身脱水，手術施行と身体への侵襲が重なり，術後全身合併症の発生が多いので注意を要する。このような合併症の発生を防ぐには，薬物療法やlaser iridotomyなどで一時的寛解により全身状態の回復をまって手術を行うのがよい。

（6）膨隆水晶体の診断には，起音波検査を行い，水晶体の厚さをみるのも有用である。

We observed 13 cases of acute angle closure glaucoma due to lens swelling in mature or imma-ture senile cataract during a 5-year period ending 1983. This incidence was roughly one-fifth of that of primary angle closure glaucoma seen by us in the same period. All the cases were elderly subjects, the average age being 76 years.

The acute glaucomatous attack occurred in asso-ciation with mature cataract in 11 instances and with immature one in 2. The anterior chamber was shallow in the fellow eye in 7 cases.
