クリスタリン網膜症の2例

Two cases of crystalline retinopathy

　眼底に閃輝性黄白色斑が認められる疾患をクリスタリン網膜症というが，この2例を報告する。症例1は15歳男子で本症の初期例として興味がある例であり，症例2は35歳女子で網膜色素変性を合併する例である。いずれも遺伝歴はない。

　電気生理学的検査では，ERGは症例1では正常，症例2ではsubnormal,EOGは2例ともsubnormal，暗順応は，症例1ではIF常，症例2では中等度閾値の上昇が認められた。視野は症例1では正常，症例2では傍中心暗点が認められた。色覚はいずれもsubnormalであった。FAGでは症例2において，背景螢光は増強していたが，後極部に島興状の背景螢光欠損部が認められた。全身検査では異常所見は認められなかった。角膜生検では沈着物は認められなかった。

　クリスタリン網膜症は早期よりEOGの異常化がみられ，進行例の第2例では網膜色素上皮膚の萎縮像が示されることなどより網膜色素上皮の機能的，次いで構造的障害により，その排泄機構の障害がおこり網膜の代謝産物が沈着したものと考えられる。

A 15-year-old boy and a 35-year-old female were diagnosed as crystalline retinopathy. The condition in the first case was in the early stage of the disease and the second case was associated withretinius pigmentosa.

Numerous yellow-white glistening crystals were located inner and middle layers of the retina around the posterior pole in all the eyes. Elec-trooculogram was subnormal in both cases. Fluo-rescein angiography in the second case showed background hyperfluorescenee and partial loss of choriocapillaris. The biopsied cornea in the second case showed normal findings.
