血友病A患者における白内障手術

Cataract extractions in a case with severe hemophilia A

　血友病Aの68歳の男性の両眼の白内障手術を行いその結果を報告した。

（1）両眼とも手術前日より術後3日間まで濃縮第VIII因子製剤（コンファクト8®）2,000単位の投与を行い血中第皿因子活性を20％以上に行った。

（2）右眼は瞬目麻酔，球後麻酔下に角膜内切開法による水晶体嚢外摘出術を行った。

（3）左眼は瞬目麻酔，球後麻酔下にCavitron 8,000Vによる水晶体超音波乳化吸引術を行った。

（4）両眼ともWet-Field coagulatorで虹彩根部を凝固した後周辺虹彩切除を行ったがほとんど出血はなかった。

（5）両眼とも術後黄斑部浮腫を発症し視力が一時低下したが，イドメシン，およびリンデロンの投与により視力は回復した。

（6）両眼とも術後経過は良好で最終視力右＝0.04（0.7×＋7.5D○cyl−1.25 D Ax180°），左＝0.05（1.0×＋11D○cyl−0.5D AX30°）であった。

（7）以上のような血友病A患者における白内障手術を適正な第VIII因子の補充療法と超音波乳化吸引法，Wet-Field coagulatorの使用などにより術中術後にほとんど出血なく行うことができた。

Successful cataract surgery in either eye of a 68-year-old male with severe hemophilia A was perform-ed under cover of cryoprecipitated factor VIII concentrate prepared from human plasma. The level of factor VIII activity, which was below 1%, preoperatively, was maintained above 20% during surgery and the first 3 postoperative days by in-travenous infusion of 1000 units of cryoprecipitated factor VIII concentrate every 12 hours.

A planned extracapsular extraction was perform-ed in the right eye. Corneal incision was madein the clear cornea to avoid bleeding from the lim-bal vassels.
