白内障手術における術前倒乱視の転帰

Fate of against-the-rule astigmatism after cataract surgery

　超音波乳化吸引術（KPE）が嚢外摘出術（ECCE）より術後乱視度が小さいことは経験的によく知られている。また強角膜縫合糸，縫合法，結節の回数術者の技術などの因子が術後の乱視度に及ぼす効果は大きい。その乱視度は術前の乱視度や患者の年齢で変化することが考えられるが，KPEとECCEにっきそれを詳細に検討した論文は見あたらない。一人の術者が同じ縫合糸（10／0ポリエステル，マーシリン），縫合法，手術法を用いて行った408眼の手術のうち，術前倒乱視167眼の角膜乱視を6か月にわたって調べた。

　KPE, ECCEにかぎらず，70歳以下の術後角膜乱視は80歳以上の患者より強主径線が術直後はよりsteep化するが早く改善した。高齢者（＞80歳）では，強主径線の改善効果が70歳以下の者より遅延し，6か月後の乱視度は70歳以下より大きかった。ECCEでは術前1.5 D以下の倒乱視と高年齢者（＞80歳）において術前倒乱視の度数が高く，“危険因子”として考えられた。このことを倍角座標のみでなく，あらたにベクトルの動きで座標平面上で示した。

We evaluated the effect of cataract surgery onagainst-the-rule astigmatism in 167 eyes. Cataractsurgery was performed by Kelman's phacoemul-sification (KPE) in 118 eyes and by conventionalextracapsular cataract extraction (ECCE) in 35eyes. All the surgeries were performed by one sur-geon and by the same procedure. Double runningsuture was used to close the corneoscleral incisions.

There was an early and pronounced shift todirect astigmatism in both cases. In ECCE group, there was a remarkable tendency for surgicallyinduced astigmatism to increase irrespective ofpreoperative state and the age.
