重症糖尿病性網膜症に対する広汎光凝固療法の効果と障害，および適応について—1．効果と適応

Panretinal photocoagulation for severe diabetic retinopathy.:1. Effect and indication

　重症糖尿病性網膜症に対して広汎光凝固を行い，6ヵ月以上経過観察した47例76眼の成績は以下の通りであった。

（1）全症例の68.2％に有効であり，とくに前増殖期の症例や網膜新生血管のみがみられた症例には著効がみられた。（2）光凝固を短期間に完了した症例がより有効であった。（3）硝子体出血例，巨大な新生血管を有した症例には効果は不確実であった。

　このような成績にもとづいて，広汎光凝固の適応として，（1）前増殖期の症例や綱膜新生血管のみがみられる症例でも，毛細血管床閉塞領域が広い場合や他眼がすでに増殖性病変を示している時には適応がある。（2）乳頭上新生血管がみられる増殖型網膜症は絶対的適応がある。

We treated 76 eyes of 47 cases with severe dia-betic retinopathy with panretinal photocoagula-tion using argon laser with a follow-up period of 6 to 24 months.

The retinopathy improved in 52 of 76 eyes (68%). Particularly favorable response was ob-tained in eyes with preproliferative and early pro-liferative diabetic retinopathy. More favorable response was obtained following a short-term treatment of less than 4 weeks than long-term one. The response to photocoagulation was poor in eyes with vitreous hemorrhage and large neovascula-rizations on the retina or the optic disc.
