糖尿病性網膜症の新生血管および網膜血管床に対する光凝固療法の効果について

Effects of photocoagulation for new vessels and retinal vascular bed in diabetic retinopathy

　重症型糖尿病性網膜症患者18例中27眼について光凝固を行い，その前後の螢光眼底像を比較し，毛細血管非灌流野の状態および新生血管に対する光凝固療法の効果を検討した。

（1）毛細血管非灌流野において毛細血管再造影を認めたものは27眼中20眼で，再疎通型を示したものは20眼中17眼，再形成型は14眼であった。再疎通型は光凝固による循環改善効果を示すと考えた。

（2）増殖型15眼について光凝固後その網膜新生血管の72.4％が，また乳頭新生血管は57.1％が消失・縮小し，光凝固は網膜新生血管により効果を示した。

　また以上の結果を検討して，光凝固の適応時期・手技について少考を加えた。

We evaluated 27 eyes (18 cases) with advanced diabetic retinopathy before and after treatment by focal or panretinal photocoagulation with argon and/or krypton laser. Fluorescein angiography was used to evaluate the effects of photocoagulation on the retinal vascular bed and the newly formed ves-sels.

We could observe that new vessels grew into the previously non-perfused areas in 20 eyes 1 to 11 months after photocoagulation. The vessel new-growth was interpreted as recanalization and/or revascularization in the occluded capillary bed.
