シリコンループ留置による涙道形成術

Silicone intubation of the lacrimal passages

　小児の鼻涙管閉塞で何回もブジーを通しても治らない症例があるが，これらの患者に以前は適切と思える治療法がなかった。

　最近，逆行シリコン留置術など報告されているが，今回私たちはこのような症例にGuiborのCanaliculus Intubation Setを用い，涙嚢部切開を行わずにシリコンチューブ留置し，涙道形成を行った。本法は涙道本来の経路を通り小児の顔面に傷を作らない上に，手術操作になれれば，手術時間も短くて終るなどの利点がある。また涙道に留置したシリコンチューブは今のところ長期間おいても何ら障害がみられず，刺激症状も出ないのですぐれたものと考える。

Recurrences of lacrimal obstruction is a likely event even after repeated probings. We recommend silicone intubation of the nasolacrimal duct as developed by Guibor (1975). This procedure has various advantages. By this method, traces of surgical incision are avoided. Silicone tubing is attached to light steel wire of 0.5 mm in diameter which can be easily deflected. The silicone tubes are left in the lacrimal passage.The surgical time can be kept to a minimum once the operator has become used to the proce-dure.
