Vergence-prismsによる先天眼振の治療経験

Application of vergence prisms method (Dell'-Osso) to congenital nystagmus. A case report

緒　言

　先天眼振に対する治療法としては，そのfaccturnに対し，手術（Andcrson法など）およびversion prisms装用が主に行なわれ，眼振そのものを減弱させる手段はあまりとられていない。眼振そのものを減弱させる方法としては，CüppersのFadenoperationが最近行なわれ始め，わが国でも報告^1）されている。また，三井の唱える水平筋の切断ぬいつけ術（筋を付着部で切断し，すぐ元の付着部へぬいつける）も報告^2）されている。一方，Dell'Ossoの唱えるvergence prismsによる治療法^3〜5）は，非観血的であり，簡便に外来通院で行なうことのできる方法であるが，わが国では未だ追試されておらず，報告もない。そこで今回われわれは，典型的な先天眼振の一成人男子例に対し，vergence prismsによる治療を試みたところ，視力が格段に上昇するという望外の成果を得たのでここに報告する。

A 28-year-old man with congenital nystagmus was treated with vergence prisms method (Dell' - Osso). His visual acuity was 0.5 in the right eye, 0.6 in the left eye and 0.4 with both eyes. The nystagmus was pendular horizontal and had null point at 20Δ. of right lateral gaze. The binocular function was in grade III. With vergence prisms, 7Δbase-out prisms, his binocular visual acuity improved up to O. 8, and successfull results were obtained.

In discussion, the role of convergence on the suppression of nystagmus with dramatic improve-ment of visual acuity was mentioned.
