網膜静脈閉塞症における視力予後—黄斑部微小血管系との関連について

Correlation between visual outcome and extent of microvascular involvement in the macula in retinal vein obstruction

緒　言

　網膜静脈閉塞症^{6，7，10，16，21，22，24，25，28）}は，しばしば重篤な視力障害を来たすが，その予後は一定せず比較的良好な視力に回復する症例がある一方，高度の視力障害を残すものもある^{1，9，13，19，22）}。そこでもし本症の初期の所見より，その視力予後を推定する事が可能ならば，臨床上の価値は大きいと考えられる。

　従来，本症における視力低下は主に黄斑部網膜の障害程度に左右されるものである^{1，7〜9，27，28）}事が知られており，その結果として生ずる初発時の視力障害の程度^12）や，閉塞静脈と黄斑部との位置関係^4，9）等が，視力予後に関係する因子であるとされて来た，また近年Clemettら^3）は，螢光眼底造影の所見より，特に中心窩をかこむcapillaryarcadeが完全に保存されているか否かが，視力予後を考える上で重要であると報告した。

We evaluated the correlation between long-term visual prognosis and the extent of invovelment of the microvasculature in the perifoveal area in 4 eyes with obstruction of the central retinal vein and 11 eyes with branch vein obstruction. All the cases were initially studied by means of fluo-rescein angiography and were followed up at least for one year. We noted a positive correlation between the one-year visual acuity and the extent of preserved micro-vasculature in the macular area.
