Report of 4 cases of congenital hypertrophy of the retinal pigment epithelium

緒　言

　日常の診療で眼底の色素性病変をみた場合，悪性黒色腫との鑑別が大切である。鑑別上重要なものの一つに，congenital hypertrophy of the reti-nal pigment epithelium （以下CHRPEと略す）がある。CHRPEは，網膜色素上皮の良性病変で，普通孤立性，円形，扁平，境界鮮明で黒禍色調を帯びている。過去には，悪性黒色腫と誤診され眼球摘出を受けた例で報告^1〜4）されてきた。Re-eseとJones^1）は，悪性黒色腫の疑いのある151眼の中に，片眼性，孤立性，黒色調，境界鮮明，扁平，円形ないし卵円形の眼底病変を9眼見出した。病変は網膜色素上皮にあり，組織学的に色素上皮が過度の色素を含有していることをつきとめ，benign melanoma of the retinal pigmentepitheliumと命名し，限局した色素上皮の先天性発育過度による病変と考えた。またReese^5）は，hyperplasia of the retinal pigment epitheliumと記載したりした。KurzとZimmerman^3）は，同様の臨床像を呈し眼球摘出された眼で，組織学的に網膜色素上皮の肥大であることを示し，conge-niしal hypertrophy of the retinal pigment epithe-liumと命名した。

Four cases of congenital hypertrophy of the re-tinal pigment epithelium (CHRPE) were reported. In all cases, subjective symptoms corresponding to this lesion were absent. There was nothing in particular in the past or family history. None had evidence of other ocular diseases. All cases were unilateral, and the solitary, flat, hyperpigmented and well demarcated lesion was present. A few of the characteristic findings of CHRPE which are so-called peripheral marginal halo, peripheral mottl-ing, orange mottling and "swiss cheese" mottling were observed in each case.
