網膜剥離手術による眼球の容積変化と形態変化—その1 強膜短縮術について

Changes of the volume and the shape of the eyeball through retinal detachment surgery: I. Scleral shortening

緒　言

　強膜短縮術を眼球の半周以下の範囲に行うと強膜が短くなり眼球容積も縮小する割には術前術後の屈折の差が少ないことが臨床上経験される。手術時被手術眼を詳細に観察すると強膜短縮術による変化には，眼軸の短縮もなくはないがそれ以上に眼球壁の一部が眼球内に陥入することや，手術部周辺で眼球壁が球面から平面に近く形態変化することが重要な要素であるように思われる。すなわち，強膜短縮術による眼球容積変化と形態変化は単に短縮幅に見合う眼軸の縮小として計算できるものでない。しかし手術によつて生ずる眼球容積変化と形態変化を正確に知つておくことは前述の臨床的観察を定量的に裏づけ，同時に強膜短縮の効果と適応を理論づけるために是非とも必要なことである。

　そこで著者らは（1）強膜短縮術による眼球の容積変化，（2）同手術による眼球の形態変化，（3）実際に同手術を受けた症例の屈折変化の3点について，人眼ならびに豚眼を用いた実験と自験症例の屈折測定結果をもとに比較検討し臨床的に有用と思われる結果を得たのでここに報告する。

Lamellar undermining method of scleral shorten-ing was experimentally carried out in 8 human and 20 pig eyeballs.

Volume changes of the eyeballs through sur-gery were determined from the difference bet-ween preoperative and postoperative weights of globes measured under an equally adjusted intra-ocular pressure.

Changes of the shape of an eyeball were deter-mined through differences of three diameters ofthe eyeball: one antero-posterior and two equa-torial diameters, one of which was parallel to the operative field and the other vertical to that.
