網膜剥離手術（鞏膜短縮術）

Scleral Shortening Operations with Special Reference to Scleral Lamellar Resection

I．緒言

　世に温故知新ということばがあるが，これが網膜剥離に対する鞏膜短縮術についてほど，ぴつたりすることは他にあまり例がないように思われる。すなわち，すでに多くの学者の述べているとおり，1903年L．Müllerが帯状に鞏膜を切除縫合し，眼球を短縮して網膜剥離を治癒させようと企てたが，学界の注目をひかぬ間にGonin （1916）の裂孔閉塞法がはなばなしくデビユーし，1933年Lindnerがふたたびこの方法をとりあげたときも．「当時の大勢は，普及の途についたばかりの裂孔閉塞術の華々しい成績と，その把握への専念の結果」（百々^1））ほとんどかえりみられることなく，また20年近い歳月が流れた。しかし裂孔閉塞術の普及と共にその奏効限界が知られるようになり，網膜剥離の手術療法の進歩もようやく1つの壁にぶつかつたように見えた。ここにおいてその打開策の1つとして鞏膜短縮術がまたまた脚光をあびるにいたつたのである。

Besides a review of Japanese literature on scleral shortening operations a oreport is made of 24 cases (24 eyes) of retinal detachment which have been operated with lamellar resec-tion of the sclera.

1) Sex : male 13 and female 11.

2) The patients operated range from 14 to 62 in age. They are for the greater part 21 to 50 years of age.

3) Affected eye : 14 right and 10 left eyes have been affected.

4) Interval of onset of the disease and the operation range from 18 days to 2 years and 4 months. to 13 cases the eye has been operated in 3 months after the onset of the disease.
