We evaluated changes in refractive elements in 125 eyes of retinal detachment treated by scleral buckling procedure. Postoperative myopinization recovered after 4 years of surgery in 20 eyes treated by exoplant. Myopinization of about 1 diopter persisted for 10 years or longer in 105 eyes treated by scleral implant. Corneal refractive power increased within one year after surgery averaging 0.9 diopter in eyes receiving exoplant. It was greater in implant eyes without posterior vitreous detachment (PVD) . Corneal astigmatic vector averaged about 2 diopters immediately after surgery with exoplant. In eyes receiving implant, vector of about 1 diopter persisted for 6 years or longer. It was more pronounced in eyes with PVD. The findings show that surgery with exoplant induces temporary myopinization with more pronounced corneal changes immediately after surgery. Surgery with implant results in lasting buckling effect and myopinization for 10 years or longer.
