A 51-year-old man with diabetic retionpathy and impaired vision in both eyes was examined by fluorescein fundus angiography using 5ml of 10% fluorescein solution. He felt numbness in the upper extremities and dysarthria appeared about 35 minutes after the examination. At that time the blood pressure in supine position was 194/122 mmHg in the left arm. About 3 hours after the examination he became comatose and died 37 days later.

An autopsy revealed an extremely sclerotic stenosis of basilar artery and brain stem infarc-tion. He had diabetes mellitus for 20 years and had been a known hypertensive for the last 10 years.
