網膜静脈閉塞症における線溶療法後の螢光眼底所見

Fluorescein Angiographic Study of Retinal Vein Obstruction Treated by Enzymatic Fibrinolytic Activator

緒言

　網膜静脈閉塞症の治療目的は，まず第一に中心視力の改善であることはいうまでもない。視力に影響する黄斑部の浮腫および出血は，いずれはなんらかの機転により吸収されるにせよ，吸収に長期を要すれば視機能の回復は悪い。著者らは1973年の第27回日本臨床眼科学会において，網膜血管閉塞症に対してurokinaseの早期連続投与により，出血と浮腫の吸収，視力や網膜循環状態の改善が著明であることを報告した^1）。今回は網膜静脈閉塞症の30症例に対して，urokinase療法前および後の数回にわたり高速連続螢光眼底撮影を行ない，網膜血管所見，特に閉塞部近くの状態と，出血や浮腫の吸収過程について検討し，どのような症例に光凝固の適応があるか考えてみたい。

Thirty cases with retinal vein obstruction were frequently examined by serial fluorescein fundus angiography before and after treatment with retrobulbar urokinase for a period ranging from 2.5 months to 17 months.

Five cases out of 6 with central retinal obst-ruction showed normal vision and an almost normal pattern of retinal vessels by fluorescein fundus angiography after treatment. Four of these 5 cases demonstrated dilatation of radial peripapillary capillaries in the early stage after treatment.

Twenty-three cases out of 24 with branch vein obstruction showed improvement of vision after treatment.
