網膜色素変性症に合併した緑内障について

Glaucoma Associated with Retinitis Pigmentosa

緒　言

　網膜色素変性症に合併した緑内障については古くから多くの報告があるが，その多くは1例または数例の症例報告にとどまり，本態にまで言及した論文は少ない。また，両疾患の合併についても，それを単なる偶然によるとする説がある一方，網膜色素変性症には何らかの理由で緑内障を合併しやすく，その原因を両疾患の発生に関する遺伝子の近接に求める説等もあり，種々の推定がなされてきたが，結局明確な結論は出ていない。

　両疾患の合併に関する論文は最近10年間は内外ともに非常に少なかつたが，近年，網膜色素変性症の房水動態についての検討がなされ^1），また網膜色素変性症の隅角につき，統計的観察が行なわれる^2〜4）等，網膜色素変性症と緑内障との関係か再び注目されている。

Seven cases of glaucoma associated with retinitis pigmentosa are described. Of these 7 cases, 4 had simple glaucoma and the remaining 3 had chronic congestive glaucoma. Goniosco-pically, 10 of the 14 eyes showed narrow angle.

In spite of maximum medical therapy, control of intraocular pressure was poor in all the eyes. They also responded poorly to repeated surgery. Glaucomatous disc cupping did not develop in 3 of the 4 patients with simple glaucoma in spite of persistent elevated intra-ocular pressure for 3 to 10 years.
