高血圧患者に於て脳出血及び視束出血とともに発来した網膜黄斑部の円盤状浮腫—其の臨床所見と剖検所見

Disciform macular edema and optic nerve sheath hemorrhage in simultaneous occurrence with apoplectic stroke: Their clinical and histological appearance

緒言

　ここに報告する症例は中等度の動脈硬化性網膜症を有した男子の患者で，九大眼科の外来に通院中，待合室に於て突然脳出血の発作を起して死亡した。私共は其の患者の死の半時間前に眼底を検査し，左眼の網膜黄斑部に漿液性中心性網膜炎に酷似する円盤状浮腫巣を発見した。死後其の眼球を剖検し，此の浮腫が中心窩の前面に貯留した網膜前滲出液であり，黄斑部毛細血管網より出たものであることを確かめ得た。従来「血管痙攣性中心性網膜症」の名称の下に呼ばれる疾患の病態はあいまいであるが^1）5）8），この症例の病態こそ，この名称に適合したものと考えられるので，其の臨床所見と剖検所見をここに記載する。

The case to be descrived below was a 43-year-old male, who used to play a daily visit to our clinic to get a prosthesis prepared for his right eye, which became phthisic. Hisleft eye had arteriosclerotic retinopathy with a few punctate hemorrhages in the peri-pheral part of the retina, though it was practically normal in visual acuity, with its macular area left unaffected. On May 16, 1960, when the patient was waiting his turn in the waiting room, he had a sudden attack of cerebral hemorrhage and fell senseless to die 5 and half hours later.
