網膜剥離に対する鞏膜短縮術（仮称Chamlin-Rubner法）の経験—予報．附 鞏膜短縮術の適応に関しての私見

Modified Chamlin-Rubner's Lamellar Underminig, a technique of scleral buckling, for retinal detachment. (A preliminary report).

　網膜剥離に対する鞏膜切除術（鞏膜短縮術，眼球短縮術，鞏膜切除短縮術）の効果が，はつきりと我国に於ても認められて来た事は，臨鉢眼科14巻3号の網膜剥離特集をみても之をうかがい得る。勿論この術式を行うことに慎重な人々と，積極的に適応をひろげて行う人々とがあるが，鞏膜切除術が，単純なるジアテルミー凝固法では救い得ない症例群に対して，何割かは有効である事は，大部分の術者の認めているところである。残された問題は，鞏膜切除術の適応をよりはつきりさせる事と，術式の改良及び本術式の有効限界を明かにする事であろう。

　私は昭和35年3月以降，1956年Chamlin及びRubnerが記載した方法^1）（Lamellar under-mining）を採用し，之を私の鞏膜切除術に対する考えに則つて術式を若干改めた変法によつて鞏膜短縮を行つている。症例が少なく，術後観察期間も充分でないが，予報として報告し，あわせて鞏膜短縮術の適応に対する私見を述べる。

The author modified Chamlin and Rubner's lamellar undermining (1956) for retinal de-tachment. In this paper, the technique of the author's scleral buckling was introduced and his results were shown as a preliminary report. Six, including 4 cases where simple diather-my or lamellar scleral resection had been done elsewhere without success, were operated by his method. In 5 out of these 6 cases, the detached retina was cured, and in another case the retina was reattached, but vitrous hemorrhage occured 33 days after operation. The author discussed on the indication of scleral resection in a detached retina.
