Neptazane （Lederle）の臨床的・実験的使用経験

Clinical and Experimental studies with the Methazolamide (Neptazane)

緒言

　炭酸脱水酵素Carbonic Anhydrase （以下C．a．と略す。）がHCN，H₂S等に依つて其の作用が阻害される他に，Sulfonamideに依つても抑制される事は以前からも知られていた。近年Acetazolamide〔Diamox-Lederle〕（以下A．a．と略す。）がC．a．抑制の目的を以つて合成され，其の顕著な利尿作用の故を以つて斯界に登場した。A．a．の作乍用は利尿のみに止まらず，1954年Becker氏等に依り緑内障眼に対して降圧作用のある事実が報告された。以来，之に就いて陸続として臨床的或は実験的研究が発表され，作用機序其の他に関して幾多の問題を提示したが，臨床的には緑内障治療に於いて極めて高い地位を占めるに至つた。併し僅かではあるが副作用が認められる為にその後更に製剤の改良が試みられている。炭酸脱水酵素抑制剤（以下C．a．i．と略す。）の作用基はSulfonamide-基であろうとの推測の下に各研究所に於いて，Methazolamide,Ethox-zolamide, Dichlorphenamide, Chlorothia-zide, Nirexon等次-とSulfonalnide-基を有するC．a．i．が合成，発表された。何れもSulfon-amide-基1〜2箇結合しており，C．　a．抑制作用はin vitroでは強力であつたと発表された。又大方は利尿作用を発揮する。

Acetazolamide (Neptazane) was administered to glaucoma with good results. Neptazane (100-200mg) seemed to have the same hypotensive effect as that of Diamox. Onset of the action of Naptazane was slightly slower and duration was longer than Diamox. No side effect was observed.

Tonography and laboratory findings (transport of fluorescein to anterior chamber and chemical examination of serum and aqueous humor etc. ) suggested that the mode of action of Neptazane was due to depression on the production of aqueous humor.
