網膜剥離の治癒見込推定に関する考究—其の1 術前の安静による網膜剥離自然減少の有無と手術成績—其の2 手術中乃至終了時の剥離減少の有無と手術成績

Investigations into Precognition of Prognosis of Retinal Detachment :Results of Surgical Treatment and Changes in the Degree of Detachment :During Preoperative Rest as well as During Surgery

緒言

　Goninによつて網膜剥離に対して裂孔閉塞手術が初めて打立てられてより以降の，網膜剥離手術成績に関する多数の統計的観察の結果は，観点を転じて見れば，とりもなおさず手術予後を左右する条件に関する一定の原則を求めるべく検討したものとも見ることが出来る。我々が多数の網膜剥離を扱つて来た経験を回顧すると，個々の例に於いて，一見治り難くそうに見える剥離で，手術によつて迅速に容易に復位したものもある一方，非常に治り易そうに見えるもので，予想に反して治り難いものに遭遇することがある。個々の例について正確に手術予後を予言することの困難性は，Goninの著書^1），Mellerの手術書^2）にも指摘されている。しかしながら次に列挙する項目は，予後不良を推定せしめる条件として，衆論の一致するところである。1）発病後長期を経た古い剥離，2）網膜の強い萎縮変性，皺襞形成，結合織増殖等剥離した網膜自体に強度の変化を伴うもの，3）巨大裂孔，並に多数の裂孔又は類嚢胞変性が広汎に認められるもの，4）網膜剥離が高度広大で，臥床安静又は網膜下液穿刺排除にて減少が著しくないもの，5）無裂孔剥離，6）硝子体変化の強いもの，7）無水晶体眼。

It has long been pointed out that the more extensive the flattening of the retina during preoperative rest or during surgery the better is the prognosis. This relationship is, how-ever, not yet fully elucidated with statistical proof. The author has attempted to clarify this problem, and has made statistical investigations of patients whose eyes suffered reti-nal detachment and were operated on by means of diathermy alone or combined with scle-ral resection at our clinic during the period from January 1953 to December 1957.
