頭蓋内圧と眼底血圧との関連についての一考察

Consideration of relation between the cerebrospinal fluid pressure and retinal blood pressure

緒言

　眼底血圧と髄液圧との関係についての文献は本邦において極めて少ないが，西欧には比較的多く見られ，脳腫瘍・髄膜炎等頭蓋内圧上昇性疾患の場合に眼底血圧が上昇する事を認めている報告が多い。しかしながらかかる疾患においては，正常者高血圧症患者等における眼底血圧と髄液圧との関係とは事情を異にすると考えられるので，これらのものを綜合観察するのでなければ，果して脳腫瘍，髄膜炎等において眼底血圧に変化が生ずるか否か，またもし生ずるならば，それがいかなる意義を持つものであるかについては明瞭でない訳であり，現在未だかかる域に達してはいない。元来脳腫瘍，髄膜炎等における髄液圧と眼底血圧との関係を研究する目的は，髄液圧脳圧の上昇が直接にまたは脳循環への影響を介して間接に眼循環に影響するかどうかを知る事にあると考えられるが，近年髄液圧と脳循環との関係についての研究が発展し，脳腫瘍，髄膜炎等における脳循環の状態は次第に解明されつつある。一方眼底血圧測定の意義についてWeigelin等によつて唱えられている説即ち眼底血圧は眼内局処の血圧を現わすものでなくて，更に深部の眼動脈血圧を現わしており，この眼動脈血圧は頭蓋内の血管抵抗を反映するという考え方が本邦に紹介された。

A male of 26 years old, with atrophy of the optic nerve in the right eye and neutritis optica in the left eye and without any hypertensive symptom, showed a change in medul-lary liquor pressure during the admission. The maximum value was 600mmH₂O and no stasis papillaris was observed. His blood pressure of the eye ground before lumbal punction was measured with "Keio" type ophthalmodinamometer. A slight increase in blood pressure of the eye ground parallel to the increase in medullary liquor pressure was found, but the arterial blood pressure of the upper arm showed no parallelism with the medullary liquor pressure.
