緑内障の治療（第Ⅰ報）—ピロカルピンに就いて

I．緒論

　ピロカルピンは，緑内障に用いる最も普通の薬剤であるが，これに対する私共の智識は案外に少い様である。これは通常，朝昼晩という風に，1日数回用いられている。しかし，私共は，実際にこの様な方法で，殆んど効果の無かつた者が，一時に数回，衝撃的に点眼して，著しく流出率が増し，眼圧下降を来たした数例を経験し，点眼方法による効果の違いを検討する事にした。又，一方ピロは濃度を高めて10％迄用いられているが，Sugarは2％以上の濃度にしても，効果はあまり変らないと述べている。しかし，これ等の点について，参考となるべき記載は見当らない。私共は，ピロの1％1回，10分（或は5分）間隔で4回，4％を3分間隔で2回，という3つの点眼方法を用い，緑内障に於ける眼圧下降の状況を追求した。尚，同時に，瞳孔径，球結膜の状況，Tonographyによる前房水の流出率，産生量等をも調べた。緑内障は個々の症例によつて，薬剤の効果が異るので，その症例の大要は「緑内障の視野」という題目で，別に発表する予定である。

Thirty cases of simple glaucoma, 4 cases of chronic congestive glaucoma, 3 cases of ecute congestive glaucoma, and a case of Sturge-Weber's disease were administered Pilocarpin by three methods : The first method was by giving a 1% solution once, the second was a 1% solution 4 times at intervals of 5-10 minutes, and the third method was by giving a 4% solution twice at 3 minutes intervals

The fall of ocular tedsion was observed. The results are as follows : The latter 2 meth-ods lowered the ocular tension more strongly than the former.
