隔日に視野の変動する緑内障の2例—第3報薬剤による視野狭窄の変化

Two cases of simple glaucoma with daily variations of isopters. The effects of medication to visual field loss in simple glaucoma

I．緒言

　前報で一日おきに視野が広くなり，狭くなりする緑内障の原因について検査したところ，眼圧と全身〜眼内血管圧との不均衡によつて，視野の変動する事が判つた。そこで眼圧あるいは血圧を変動させる薬剤を用いて，視野がどのように影響されるか調べてみた。この検査成績より治療法を考案し，狭い日の視野極小になるのを防ぎ，眼圧下降と同時に，視野を拡大させることが出来て，良好な経過を収めているので，これについて述べる。症例の詳細については第1報で述べたので，大略のみ記すと，症例1は32歳女子，症例2は54歳男子，何れも両眼眼圧亢進著明，流出率極小，視野，眼底変化強く，狭隅角で，全身血圧低く，眼圧血圧比が小さい。視力は第1例は右眼正常，左眼0，第2例は右眼50cm手動，左眼0であつた。眼圧は，第1例は左眼手術によつて，右眼は薬剤を用いてほぼ正常となつた。第2例は両眼共手術によつて非常に低くなつた。検査は全て薬剤を48時間以上中止して行つた。

The author used subcutaneous injection of 0.1% Adrenalin (0.5cc), instillation of 0.1% Adrenalin solution and subcutaneous injection of 0.5mg Apoplon (Reserpine) to 2 cases of glaucoma with visual field loss fluctuatingevery other day, and measured the varniatio in intraocular pressure, brachial and retinal blood pressure and visual field defects. By giving Adrenalin injections or by eye drops the blood pressure rose and the intraocular pressure decreased.
