血圧と関係ある2，3の眼底所見について—其の3 高血圧性眼底所見判定上の疑義について

緒言

　高血圧患者の眼底所見判定は，所見有無の判定から所見の程度及び性格（良性・悪性）の判定を要求せられる時期へ移行し，判定の機会も次第に多くなつてきた。而もこの種判定は通常生命予後に関する暗示を含み，患者の心理と内科主治医の治療方針へ著しい影響を与えるばかりでなく，高血圧を対象をする幾多の臨床的研究の根柢となることが少なくない。従つて個人により又病院や大学により判定が動揺することは，なし得る限り避けたいところである。

　然るに動脈硬化性変化を含めての眼底の高血圧性変化とは，言うまでもなく，軽度より高度へ，良性より悪性へ連続的移行的所見であり，これを微細な所見に基いて段階をもうけ区分するのであるから，同一の眼底に対する判定が動揺することは，決して無視し得る程度に止まらないように思われる。斯くして1人の患者が某医師又は某病院眼科で眼底正常と言われて安心し，他の医師又は他の病院でKeith-Wagener （以下K-Wと略称）Ⅰ群時にはⅡ群等と言われて驚くというような事例も稀ではないように思われる。

The author examined the cases of which criteria in the diagnosis of hypertensive fundus picture were at stake, and expressed the importance of the criteria, and further indicated the doubtful points on Keith-Wagener's classification and observed them on the following points.

1. Judgement on the person with normal fundus : relation between the person with normal fundus and Keith-Wagener's group I was discussed from the aspect that Keith-Wagener'sclassification will be the classification of fundus change.
